Requests for medical records may be sent by fax or by mail to the contact information on this page.
Please include patient’s name, SSN and DOB, what specific information is requested, a duly executed HIPAA release, and a fax number for our response. Please allow 15 days for our response. You will receive an invoice if the requested records are located. Records will be faxed after receipt of payment.
